Q1. CASE STUDY
Today is Monday, November 16th 2009, Dr. Agatha woke up at about 7.30 a.m.,
already late for breakfast. She had a clinical meeting at 8.00 a.m. She dashed to
the bathroom, had a shower and dressed in a hurry. That gave her no time to
prepare herself and she arrived late.
During the meeting, she had to think about what she had to do during the day.
She did not contribute much. After the meeting she continued to think about the
rest of the day. It took her 30 minutes to put down on paper some of her activities
during the day.
• Meeting a visiting team from the Provincial headquarters.
• Patient consultations.
• Surgical operations (2 patients).
• Writing a report to the Provincial medical Officer on an outbreak of cholera.
• Hospital Management Committee at 3.30 p.m.
• Teaching for an hour at the Nurses Training School.
• Correspondence.
• Finishing reading the book on AIDS which she bought during my last visit to
Nairobi.
Which one should she table first? All seemed to be important. After wasting
another few minutes she decided to go to the theatre although the cases were non-emergency.
The time was 9.15 a.m. In the middle of the first operation the
Hospital Administrator, Mr. Kilonzo, sent a message that the visitors from the
Provincial Headquarters had arrived. She asked that they should be shown into
her Office. She finished the operation thirty minutes later and decided to
postpone the second operation.
Rather than delay the visitors, she did not write up the notes of the patient she
operated on. For the next one hour, she discussed with the visitors her hospital’s
request for the new beds and mattresses for the children’s ward. She had not
conducted the ward rounds. So she handed the visitors over to Sr. Odero to take
them for refreshment.
She went into her consulting Office at 11.30 a.m. and spent the next hour
attending the patients. She conducted the ward rounds at 12.30 p.m. During the
ward round she remembered another very important meeting with the D.O. Mr.
Kibiego and a group of councilors to discuss the issue of cholera and the sanitary
conditions in town. The District Public Health Officer, Mr. Atek had scheduled
the meeting at 5.00 p.m. She finished the ward round at 1.30 p.m. She went
back to her consulting office but an old colleague Ms. Wambugu interrupted and
she spent the next ten minutes chatting before she sent her to her house. She
decided to go for lunch at about 2.30 p.m. A lot more patients were waiting.
She resumed consultation at 3.30 p.m. By then she realized that she had missed
her teaching commitment at the Nursing Training School and had not apologized.
She saw the patients for half an hour and arrived at the Management Committee
meeting shortly after 4.00 p.m. The other members had been waiting since 3.30
p.m. They were only half way through the agenda when it was 5.00 p.m. and had
to end the meeting abruptly. She dashed to the D.O’s office but she was late. She
arrived home at about 6.30 p.m. She was tired and frustrated and she did not want
to know what she had achieved that day. She decided to postpone reading the
book on AIDS and she said to herself “I will do the rest tomorrow”.
