Intelligent as we may be, we may miss the correct answers. One factor that could militate against our ability to arrive at the correct solution is a phenomenon known as confirmation bias. This refers to the reluctance to seek information that might disapprove our beliefs. The other factor is fixation – the inability to see a problem from a new perspective. Once we incorrectly represent the problem, it is hard to restructure how we approach it. Past success can help solve present problems but it can also interfere with our finding new solutions. This tendency to repeat solutions that have worked in the past is a type of fixation called mental set. Another type of fixation is functional fixedness – the tendency to perceive the functions of objects as fixed and unchanging.

A person may for instance look for the vehicle that normally carries the sick, not realizing any other vehicle could also serve the purpose. In this fixedness, a person whose life could have been saved is lost.
