It shows all the vaccines that the child will be given and the method in which it is given.

(ii) The schedule of vitamin A supplementation.

(iii) Showing where the child's family lives such as the location or division.

(iv) The child's history.
This will include the date of birth, weight at birth and order of birth.

(v) The child's health history such as many special deformities the child might have.

(vi) The child's growth chart.
It consists of a graph onto which the child's weight is plotted every time the child visits the clinics.
It enables one to see at a glance if the child is growing well.

(vii) The child's health card.
It indicates the reasons that may call for special care, such as occurrence of childhood diseases like diarrhea.
